Job summary

Apploi is seeking a Registered Nurse to lead hospice care planning and delivery in home and community settings. The role emphasizes nursing expertise, patient/family assessment, and collaboration with physicians and the interdisciplinary team.

The ideal candidate will have RN licensure in good standing and relevant hospice or home health experience, with strong communication, care planning, and patient education skills.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in good standing to practice.
• Minimum one year of nursing experience within last three years, or BSN with two years' experience in public health/home care/hospice.
• Ability to lift and transfer patients; prolonged walking/standing.
• Valid driver's license with insured vehicle.

Responsibilities

• Completes initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises plans of care with patient and family.
• Coordinates with physicians and interdisciplinary team.
• Participates in on-call duties and ensures safe patient environments.
• Documents care and communicates changes to the physician.
