Qualifications

Responsibilities

• Conduct comprehensive patient assessments to identify needs and develop care plans.
• Provide skilled nursing care including pain and symptom management and education.
• Coordinate and supervise hospice aides and other professionals in patient care.
• Communicate progress and plans with patients, families, and the healthcare team.
• Monitor well-being and comfort; align care with patient goals and wishes.
