Job summary

Paycom is seeking a Hospice Registered Nurse Case Manager to deliver professional nursing care to home care patients and, as needed, in institutional settings. The role requires an Oklahoma RN license, hospice experience, and strong communication.

The RN will conduct admissions, routine visits, and supervisory duties, while maintaining comprehensive records and coordinating with an interdisciplinary team to manage care plans and ensure compliance with Medicare guidelines.

Qualifications

• Must hold a current Registered Nurse (RN) license in Oklahoma.
• Minimum of two years hospice nursing experience preferred.
• Certification in Hospice and Palliative Nursing is highly desirable.
• Strong verbal, written, and electronic communication skills.
• Ability to adapt to frequently changing tasks and priorities.
• Must possess a valid driver's license and reliable transportation.

Responsibilities

• Provide direct nursing care to assigned hospice patients across settings, aligning with the hospice plan of care.
• Conduct admissions, routine nursing visits, and on‑call duties as assigned.
• Maintain and update patient records per Medicare and agency standards.
• Collaborate with interdisciplinary team and participate in meetings.
• Educate patients and caregivers on disease processes and care goals.
• Oversee supervision for LPNs and support staff as needed.
• Ensure timely and accurate documentation and incident reporting.
