Spanish Cove Retirement Village is seeking an experienced Registered Nurse (RN) Hospice Case Manager for our Home Hospice department in Oklahoma. You will coordinate patient care, collaborate with the interdisciplinary team, and ensure compassionate, quality nursing services in a home setting.

This role emphasizes adherence to hospice standards and ongoing professional growth. Qualifications include a valid Oklahoma RN license, CPR, and 2+ years in hospice or home health.

Qualifications

• Graduate of an accredited school of nursing.
• Oklahoma RN license in good standing.
• 2+ years in home health or hospice preferred.
• Current CPR certification.

Responsibilities

• Provide prescribed nursing care in home hospice setting.
• Collaborate with interdisciplinary team to meet patient goals.
• Develop and implement Plan of Care with physician orders.
• Document patient status, interventions and outcomes.
• Participate in on-call rotation and visits as scheduled.
