Job summary

Gentiva Hospice is seeking a compassionate RN Case Manager to deliver personalized hospice nursing care in patients' homes or equivalent settings. You will perform comprehensive assessments, follow plans of care, and collaborate with physicians, LPNs, CNAs, and social workers to ensure high-quality end-of-life care.

The role emphasizes patient- and family-centered support, education, bereavement guidance, and ensuring accurate clinical documentation in a timely manner.

Qualifications

• RN licensed to practice in the requested employment state.
• Minimum 1 year RN experience in acute care, hospice, home health, or related field.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, educating patients and caregivers as appropriate.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across interdisciplinary teams and document progress.
