Job summary

Gentiva Hospice is seeking an experienced Registered Nurse to serve as an RN Case Manager delivering compassionate hospice care in patients' homes or preferred settings. You will collaborate with physicians, LPNs, CNAs, social workers, chaplains, and other team members to implement and monitor individualized care plans.

Qualifications

• RN license in the state of employment.
• 1 year RN experience in hospice, palliative care, or home health preferred.
• CPR certification required.
• Valid driver's license and auto insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments, providing education to patients, family members and caregivers.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across interdisciplinary teams and physicians.
• Provide education on disease process, self-care, and end-of-life care.
• Document timely clinical records and ensure infection control and compliance.
