Job summary

Gentiva in Ohio is seeking a dedicated Hospice RN / RN Case Manager to lead patient care at home and coordinate with the care team. You will assess, plan, and document care while guiding families through the hospice journey with compassion.

This role requires an active RN license and CPR; hospice or palliative experience is preferred. Commitment to high-quality, compliant care and timely visits is essential.

Qualifications

• Current RN license in the state of employment.
• CPR certification required.
• Minimum 1 year RN experience in hospice, palliative care, home health, or related settings.
• Hospice/palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits.
• Deliver skilled nursing interventions and symptom management per care plans.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across interdisciplinary teams and consult with physicians.
• Educate patients, families, and caregivers on disease, care, and end-of-life options.
• Communicate condition changes to the care team for timely interventions.
• Support continuity of care and transitions in levels of care.
• Document timely in medical records and uphold infection control standards.
• Engage IDG and participate in on-call and patient support activities.
