Job summary
Gentiva Hospice is seeking a compassionate Hospice Registered Nurse / RN Case Manager to lead in-home patient care and collaborate with an interdisciplinary team. You will perform assessments, develop plans of care, and guide families through the hospice journey with education and support.
The role requires an active RN license, 1+ year of nursing experience (hospice or home health preferred), CPR certification, and reliable transport.
Qualifications
- RN license in state of employment
- Minimum 1 year RN experience (hospital, oncology, hospice or home health)
- Hospice or palliative care experience preferred
- CPR certification
- Current automobile insurance and valid driver’s license
Responsibilities
- Perform comprehensive patient assessments and reassessments in person or via telehealth
- Deliver nursing interventions and symptom management per plan of care
- Develop/update individualized plans of care with visit frequency
- Coordinate care with IDG and physicians
- Educate patients, families, and caregivers on disease, care, and medications
- Document clinical records accurately and timely
- Communicate condition changes to the care team
- Support transitions in care levels and referrals
- Participate in on-call and patient support activities
