Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead patient care at home or in other settings, guiding families through the hospice journey with compassion and expertise. You will assess, plan, implement, and educate, coordinating with an interdisciplinary team to ensure quality, compliant care.

Qualifications

• Minimum of 1 year RN experience in acute care, oncology, long-term care, hospice or home health.
• Hospice or palliative care experience preferred.
• Current RN license in the state of employment and CPR certification.
• Must have reliable transportation and driver’s license.

Responsibilities

• Perform comprehensive patient assessments and reassessments (in-person/telephonic) and educate patients, families and caregivers as appropriate.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams and consult with physicians and nurse practitioners.
• Educate patients and caregivers on disease processes, self-care, end-of-life care, medications and nutrition.
• Document timely clinical records and maintain infection control standards.
• Support home health, palliative and hospice referrals and transitions in levels of care.
• Participate in on-call and patient-support activities as applicable.
