Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to provide at-home nursing care, assessments, and care coordination for patients with life-limiting illnesses. You will partner with an interdisciplinary team to create and update personalized care plans, educate families, and support end-of-life care in home or assisted living settings.

The role emphasizes hands-on nursing, symptom management, and patient-family education, with a focus on dignity, comfort, and emotional support.

Qualifications

• Must be a Licensed Registered Nurse in the state of employment.
• Hospice or palliative care experience preferred.
• Strong assessment and care planning skills.

Responsibilities

• Perform comprehensive patient assessments and reassessments in-person or via telecommunication.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit schedules.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients, families, and caregivers on disease progression and end-of-life care.
• Document clinical records accurately and maintain infection control compliance.
• Support quality improvement, scheduling, and transitions in levels of care.
