Job summary

SouthernCare is seeking a Hospice RN / RN Case Manager to provide in‑home and community‑based nursing care for patients with life‑limiting illness. You will guide families, perform assessments, implement plans of care, and coordinate with an interdisciplinary team to ensure compassionate, quality care.

You will educate families, maintain clinical documentation, and support transitions in levels of care while upholding infection control and compliance standards.

Qualifications

• Current license to practice nursing in Alabama.
• CPR Certification required.
• Valid driver’s license and automobile insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments, with patient and caregiver education.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care including visit frequency and risk ID.
• Coordinate care across IDG, including physicians and nurses.
• Educate patients and caregivers on disease, self-care, and end-of-life care.
