Job summary

SouthernCare is seeking aHospice RN / RN Case Manager to lead patient assessments, develop and update plans of care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains. You will educate and support patients and families, ensuring compassionate, comprehensive end-of-life care.

The role requires a Registered Nurse with 1+ year of RN experience; hospice or palliative care experience is preferred. Travel to patients' homes is typical, with adherence to regulatory standards.

Qualifications

• RN qualification and licensure in state of employment.
• 1+ year experience as an RN, hospice/home health preferred.

Responsibilities

• Be the primary contact for patients and families, guiding them through hospice care.
• Deliver and document skilled nursing care per Plan of Care in patients' homes or equivalent settings.
• Perform initial and ongoing assessments to monitor patient condition.
• Collaborate with an interdisciplinary team to build care plans.
• Provide emotional, physical, and spiritual support to patients and families.
