St. Croix Hospice in Sioux City, IA seeks an experienced Registered Nurse Case Manager to lead each patient’s hospice care team and develop the Plan of Care with the Hospice Medical Director, patient, and family. You will provide day-to-day care, administer medications, and ensure accurate documentation.

Responsibilities include care coordination, patient/family education, on-call visits, and collaboration with interdisciplinary staff to improve quality of life.

Qualifications

• Currently licensed registered nurse.
• Ability to pass background checks and drug screening.
• Experience in hospice/home care preferred but not required.

Responsabilités

• Lead patient care planning and coordinate with the Hospice Medical Director, patient, family, and facility staff.
• Administer medications and treatments as prescribed; monitor and document patient condition.
