Job summary

St. Croix Hospice in Missouri is seeking a Registered Nurse Case Manager to lead each patient’s hospice care team and develop the Plan of Care with the Hospice Medical Director, patient, family and facility staff.

The RN Case Manager provides day-to-day patient care, manages medications, maintains records and coordinates with physicians and interdisciplinary teams to meet changing needs, prioritizing comfort and quality of life in home settings.

Qualifications

• Must hold a current RN license in state of practice.
• Experience in hospice/home care preferred.
• Ability to pass background checks and driving record review.

Responsibilities

• Lead hospice care team and create Plan of Care with team and family.
• Provide day-to-day patient care and administer medications.
• Maintain accurate clinical documentation and coordinate with physicians.
• Ensure HIPAA compliance and patient confidentiality.
• Support emotional well-being of patients and families.
• Arrange for equipment and supplies and participate in on-call visits.
