Job summary

Gentiva in Florida is seeking a Hospice RN / RN Case Manager to provide comprehensive, compassionate end-of-life care in patients' homes and related settings. You will assess, plan, and deliver skilled nursing care while coordinating with physicians, social workers, and chaplains to ensure individualized plans of care.

You will educate families, support emotional and spiritual needs, and maintain precise documentation.

Qualifications

• Must be a licensed Registered Nurse in the state of employment.
• CPR certification required.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management.
• Coordinate care with IDG and physicians to update plans of care.
• Educate patients, families, and caregivers on disease processes and end-of-life care.
• Maintain accurate clinical documentation and comply with infection control standards.
• Participate in on-call and patient support activities as applicable.
