Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead patient care at home or in care facilities. You will perform comprehensive assessments, deliver skilled nursing interventions, and coordinate with physicians, social workers, and the care team to implement individualized plans of care.

The role emphasizes compassionate support for patients and families, education for caregivers, accurate documentation, and participation in on-call duties to ensure seamless care delivery.

Qualifications

• Must hold RN license in the state of employment.
• Hospice or palliative care experience preferred.
• CPR certification required.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across interdisciplinary teams.
• Educate patients and caregivers on disease, care, and end-of-life options.
• Document timely clinical records and maintain infection control standards.
• Participate in on-call and patient support activities.
