Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to provide compassionate, patient‑centered end‑of‑life care in patients' homes or other settings. You will perform comprehensive assessments, develop and update plans of care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to ensure high‑quality, individualized care.

You will educate families, manage symptoms, document care, and participate in on‑call coverage as needed.

Qualifications

• Active RN license in the state of employment
• Minimum of 1 year RN experience in hospice, palliative care, home health or acute care settings
• CPR certification and reliable transportation

Responsibilities

• Lead as primary contact for patients and families, guiding them through hospice care
• Deliver and document skilled nursing care per Plan of Care in patient homes or equivalent settings
• Perform initial and ongoing assessments and adjust care accordingly
• Collaborate with IDG to refine personalized care plans
• Provide emotional, physical, and spiritual support to patients and families
• Educate caregivers on safe care practices and symptom management
• Communicate patient condition updates to the care team for timely interventions
• Maintain accurate clinical documentation and adhere to infection control standards
• Support quality improvement initiatives and care scheduling
• Navigate hospice topics such as medication administration and end-of-life processes
