Job summary

Heart to Heart Hospice is seeking an RN Case Manager to assess and address the physical, psychosocial, and environmental needs of patients with life-limiting illnesses. You will implement and adjust the care plan, collaborate with the care team, and participate in IDG meetings to ensure coordinated, high-quality patient care.

The role requires an accredited nursing degree and an active RN license, plus a valid driver's license and reliable transportation.

Qualifications

• Nursing degree required or graduation from an accredited program.
• Current State RN License required.

Responsibilities

• Initial and ongoing comprehensive assessments of the patient's needs and ADLs.
• Implement the individualized plan of care and revise as necessary.
• Coordinate with patient, caregiver, physicians and healthcare professionals.
• Attend IDG meetings and participate in care planning and prioritization.
• Provide on-call coverage and timely documentation.
• Ensure compliance with laws, regulations and policies.
