Job summary

Heart to Heart Hospice is seeking a Registered Nurse Case Manager to deliver comprehensive hospice care to patients and families. The role includes conducting initial and ongoing assessments, developing individualized plans of care, and coordinating multidisciplinary care with physicians and other healthcare professionals.

Qualified candidates are graduates of an accredited nursing program, hold an active RN license, and have strong care coordination and communication skills.

Qualifications

• RN licensure in good standing.
• Two years of RN experience preferred in healthcare/clinical setting.

Responsibilities

• Perform initial and ongoing assessments of patients' needs across physical, psychosocial, and environmental domains.
• Develop and revise the individualized plan of care.
• Coordinate with physicians and IDG to ensure integrated patient care.
• Attend IDG meetings and participate in care planning and prioritization.
• Provide on-call coverage as assigned and ensure timely documentation.
• Comply with applicable laws, Medicare regulations, and internal policies.
