Job summary

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing to practice.
• Experience: minimum one year as a professional nurse within the last three years, or two years in public health/home care/hospice preferred.
• Management experience not required; responsible for supervising hospice aides.
• Employment contingent upon references and criminal background checks.
• Excellent observation, verbal and written communication, problem solving, basic math skills per competency.
• Must be able to lift, move and position patients and equipment; suitable physical health.
• Must have a valid driver’s license and insured vehicle.
• CPR certification required (hands-on verification).

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients and families to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care as patient status changes.
• Initiate and adjust plans of care; coordinate with physicians and families.
• Provide health education to patients and families and promote self-care.
• Maintain accurate clinical notes and communication with the medical team.
• Ensure a safe patient environment and continuity of care with the interdisciplinary group.
