Job summary

Gentiva is seeking an experienced Hospice RN / RN Case Manager to lead empowering, patient-centered care in the home setting across Alabama. You will conduct comprehensive assessments, develop individualized plans of care, and coordinate with an interdisciplinary team to ensure comfort and quality for patients with life-limiting illness.

Qualifications

Active RN license in the state of employment.
1+ year RN experience in acute care, hospice, home health, or related field; hospice/palliative care preferred.

Responsibilities

Perform comprehensive patient assessments and ongoing reassessments.
Deliver and document skilled nursing interventions per Plan of Care.
Coordinate care with interdisciplinary team including physicians, LPNs, CNAs, social workers, and chaplains.
Educate patients and caregivers on disease, treatment, and end-of-life care.
Maintain timely clinical records and ensure infection control and compliance.
