Job summary

Gentiva is seeking an experienced Hospice RN/Case Manager to lead in-home patient care across Florida. You will perform comprehensive assessments, implement care plans, and collaborate with an interdisciplinary team to ensure compassionate end-of-life support.

The role requires Florida RN licensure, CPR, and experience in hospice or palliative care. Travel to patient homes is required and strong communication with families is essential.

Qualifications

• Active Florida RN license
• Minimum 1 year RN experience; hospice/palliative care preferred
• CPR certification
• Valid driver’s license and auto insurance

Responsibilities

• Perform comprehensive patient assessments and ongoing home visits.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit schedules.
• Coordinate care with IDG and physicians.
• Educate patients and families on disease, care plans, medications, and comfort measures.
• Document clinical findings and maintain regulatory compliance.
