Job summary

UnitedHealth Group's Deaconess Hospice is seeking an experienced Registered Nurse Case Manager to deliver the Plan of Care for hospice patients. You will collaborate with other professionals to implement treatment strategies that support physical, psychosocial, emotional, and spiritual well-being in patients and families facing end-of-life care.

The role requires an active RN license, reliable transportation, and strong assessment skills.

Qualifications

RN with active, unrestricted license in the state of practice (MS).
Experience in hospice or home health care preferred.
CPR certification or ability to complete within 90 days of hire.

Responsibilities

Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
Identifies patient/family physical, psychosocial, and environmental needs and reassesses at least every 15 days.
Initiates and coordinates the plan of care.
Documents problems, goals, interventions, and patient/family response to hospice care.
Collaborates with patient/family, attending physician and IDG members in daily patient care.
Instructs and supervises patient/family in self-care techniques when appropriate.
Maintains accurate clinical notes regarding patient condition.
Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition.
