Job summary

UnitedHealth Group’s Elite Hospice seeks a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients, collaborating with other professionals to support physical, psychosocial, and spiritual well‑being.

Responsibilities include initial 48-hour hospice eligibility evaluation, ongoing reassessment, care plan initiation, and thorough documentation; must maintain compassionate, patient‑centered care in home settings.

Qualifications

• Active RN license in the state of practice.
• Minimum 1 year of clinical nursing experience.
• Valid driver's license and access to reliable transportation.

Responsibilities

• Perform initial nursing evaluation to determine hospice eligibility within 48 hours.
• Identify patient/family physical, psychosocial, and environmental needs; re‑assess every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family responses.
• Collaborate with patient/family, attending physician and IDG daily.
• Instruct and supervise patient/family in self-care when appropriate.
• Maintain accurate clinical notes on patient condition.
• Provide care in diverse home settings regardless of background.
