Qualifications

• Conduct initial and ongoing comprehensive assessments of patients and families to determine hospice needs.
• Develop and revise care plans based on nursing diagnoses in collaboration with patients and families.
• Provide professional nursing care, administer medications and treatments per physician plan.
• Educate patients and families on health care needs and self-care techniques.
• Coordinate care with the interdisciplinary hospice team; participate in on-call duties.
