Job summary

Gentiva seeks a Hospice RN / RN Case Manager to deliver compassionate, in-home hospice and palliative care. You will guide patients and families through the hospice journey, coordinating with an interdisciplinary team, and providing education and support at home.

Responsibilities include comprehensive assessments, developing care plans, medication guidance, and timely documentation to ensure compliant, high-quality care.

Qualifications

• RN licensure in good standing.
• Minimum 1 year RN experience; hospice or home health preferred.
• Strong communication and education skills for patients and families.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home settings.
• Deliver skilled nursing interventions and symptom management per goals of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across interdisciplinary teams and physicians.
• Educate patients, families, and caregivers on disease, self-care, and medications.
• Report changes in condition to appropriate providers and document findings.
• Maintain timely clinical records and ensure infection control compliance.
• Support continuity of care and transitions across care levels.
