Job summary

Gentiva, a national hospice and home health provider, seeks a Manager RN in Trussville, AL to oversee patient care and lead clinical operations. You will coordinate care, mentor staff, and ensure regulatory compliance while driving performance improvements in a patient-centered hospice setting.

You will work under executive leadership, directing clinical teams, managing visits, and maintaining high standards of care delivered in patients' homes and local sites.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years of direct patient care experience, incl. 2 years in hospice/home-based care.
• Knowledge of hospice principles, regulatory compliance (Medicare/Medicaid/JCAHO/ACHC).
• Compassion for terminally ill patients and families.
• Ability to lead a diverse care team while adapting to emotional/clinical needs.
• Strong interpersonal, organizational, and leadership skills.
• Competency in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling including reassigned, missed, declined, or rescheduled visits based on clinical priority.
• Triage incoming referrals and medical concerns promptly and compassionately.
• Lead or facilitate Interdisciplinary Group (IDG) meetings and document minutes.
• Mentor, train, and provide clinical support to associates for development and compliance.
• Ensure care delivery meets regulatory standards and company policies.
• Support performance improvement initiatives and operational efficiencies at the location.
