Job summary

Gentiva Hospice in the United States is seeking a compassionate RN Case Manager to lead patient care at home or in other settings. You will assess patients, document plans of care, and coordinate with physicians, LPNs, CNAs, social workers, chaplains, and others to ensure comfort, dignity, and personalized support.

You’ll educate families, monitor symptoms, manage medications, and participate in on‑call coverage as needed.

Qualifications

• Minimum 1 year experience as an RN, hospice or home health preferred.
• Current RN license in the state of employment; CPR certification.
• Current automobile insurance and valid driver’s license; reliable transportation.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or setting of care.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care with an interdisciplinary team (physicians, LPNs, CNAs, social workers, chaplains).
• Educate patients and families on disease, self-care, medications, and end‑of‑life care.
• Document timely clinical records and ensure infection control compliance.
• Participate in on-call and patient support activities as applicable.
