Job summary

Affinity Hospice is seeking a Palliative Care Nurse Practitioner to join the hospice medical team and deliver care to patients across settings. You will collaborate with physicians, provide education, and participate in face-to-face encounters for 180-day and subsequent 60-day recertifications.

The role requires NP certification, an active RN license, CPR, and strong communication and multitasking skills. PRN schedule supports flexibility for patient care across our service areas.

Qualifications

• Current unrestricted license to practice as a registered professional nurse in the state of employment.
• Graduate of an approved Nurse Practitioner program.
• Current unrestricted Nurse Practitioner license by the Board of Nursing in the state of employment.
• Current CPR certification.

Responsibilities

• Initiates palliative care consultations or hospice treatment plans in all settings, including history, exam, and prescribing medications per state law.
• Collaborates with physicians and documents care in the EMR.
• Provides health education, counseling and referrals to other healthcare professionals.
• Provides follow-up care and reassesses treatment plans as needed.
• Maintains accurate patient records and supports reimbursement for services.
• Assists patients and families in determining goals of care.
