Job summary

CommuniCare Health Services seeks a qualified Hospice Nurse Practitioner to provide high-quality end-of-life care to patients in home settings. You will assess, educate families, and work closely with the Hospice Medical Director on care planning and chronic disease management.

Responsibilities include face-to-face evaluations, documentation, and adherence to federal/state regulations, with daily patient visits and weekly office meetings. Must have NP license and nursing Master’s degree.

Qualifications

• Valid Nurse Practitioner’s license and state licensure.
• Experience in hospice/palliative care preferred.
• Must be able to travel to patient homes daily and attend weekly meetings.

Responsibilities

• Perform face-to-face encounters with patients to assess hospice eligibility.
• Document clinical findings and provide reports to the hospice physician within 24 hours.
• Maintain accurate patient records to support the inter-disciplinary team.
• Comply with regulatory, accreditation, and company policies.
