Job summary

Heart to Heart Hospice is seeking a Nurse Practitioner to provide compassionate, patient-centered hospice care. You will educate patients and families, participate in care planning with the IDG, and collaborate with physicians to ensure high-quality end-of-life care.

Responsibilities include documentation, coordination with the care team, and strict adherence to regulatory requirements. Must hold RN and APN licenses with relevant experience in hospice or home health.

Qualifications

• Current State license as RN and APN.
• Driver's license with reliable transportation.
• Two years APN experience in clinical care, hospice or home health preferred.
• Knowledge of pharmacology, palliative care, and end-of-life.

Responsibilities

• Educate patients and families about disease, self-care, and end-of-life care.
• Assist in IDG's plan of care to ensure quality care.
• Collaborate with hospice and attending physicians on palliative care.
• Inform physicians of changes in patient needs and outcomes.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with laws, Medicare regulations, and policies.
