Beacon Hospice, Amedisys/Optum, part of UnitedHealth Group, invites you to join as a Medical Social Worker in home-based care. You will provide psychosocial support, assessment, and care coordination to patients and families, addressing emotional, social, financial, and environmental factors affecting health and recovery.

Collaborating with an interdisciplinary team, you will connect patients with community resources, provide counseling, and promote safe transitions in the home.

Qualifications

• Master's/Doctorate in CSWE-accredited program.
• Active LMSW or LCSW licensure.
• Current CPR certification.
• 1+ year of social work experience in a health care setting.
• Meets state and federal requirements for this position.

Responsibilities

• Assist patients and families with health-related financial, social and emotional concerns and assist with coping skills.
• Provide social work services in accordance with the plan of care.
• Communicate changes in the patient's clinical status to other healthcare team members and care center staff in a timely manner.
• Educate the patient and family members and other team members on medical social work interventions and treatment plan.
• Perform care planning, transfer and discharge functions.
• Identify risks or threats to the patient's health in the patient's environment and promptly implement appropriate interventions to reduce risks; communicate with the physician and healthcare team.
• Other duties as assigned.
