UnitedHealth Group is seeking a Medical Social Worker to provide psychosocial support, assessment, and care coordination for patients and families in the home setting. You will collaborate with the interdisciplinary team to connect patients with community resources, provide counseling, and promote optimal outcomes.

Primary responsibilities include assisting with financial, social and emotional concerns, educating families, and coordinating care transitions.

Competenze

• MSW or Doctorate from CSWE-accredited program.
• Active LMSW or LCSW licensure.
• Driver's license and access to reliable transportation.
• Current CPR certification.
• 1+ years of social work experience in a healthcare setting.

Mansioni

• Assists patients and families with health-related financial, social and emotional concerns.
• Provides social work services in accordance with the plan of care.
• Communicates changes in clinical status to the healthcare team and care center staff.
• Educates patients and families on social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks in the home and implements interventions to reduce them.
