Job summary

The position requires a MSW (CSWE-accredited) with active LMSW/LCSW licensure, CPR certification, and at least 1 year of healthcare social work experience.

Qualifications

• Requires MSW degree from an accredited program.
• Active LMSW or LCSW licensure.
• CPR certification required.
• Minimum 1+ year of social work in healthcare.
• Driver’s license and reliable transportation.
• Complies with state and federal requirements.

Responsibilities

• Provide psychosocial support, assessment and coordination for home-based patients and families.
• Collaborate with interdisciplinary team to connect patients with community resources.
• Counsel patients and families and facilitate care transitions.
• Educate on social work interventions and treatment plans.
• Perform care planning, transfers and discharge functions.
• Identify risks in patient environment and coordinate interventions.
