Job summary

Heart to Heart Hospice is seeking an Intake Coordinator to direct daily patient referral and intake operations. You will initiate charts in the EMR, maintain census data, and ensure compliance with CHAP standards and regulatory requirements.

The role emphasizes establishing positive referral source relationships, reporting referral statistics, and supporting strategic planning to meet unmet client needs. A healthcare degree and relevant experience are preferred.

Qualifications

• Degree in health care or related field preferred.
• 3–5 years experience in health care, preferably in hospice operations.
• Admissions/Intake and customer service experience.
• Valid Driver's License and Automobile Insurance.
• Knowledge of CHAP standards and private payer practices.
• Experience with EMR software.

Responsibilities

• Direct all daily patient referral and intake operations including establishing intake policies.
• Initiate chart into EMR and maintain workflow.
• Maintain census, pending admissions, red flags, journey's end and care plan meetings on stand up board.
• Ensure compliance with state, federal and CHAP requirements.
• Build relationships with referral sources to enhance perceptions of Heart to Heart Hospice.
• Gather, collate, and report referral statistics.
