Job summary

Heart to Heart Hospice is seeking an Intake Coordinator to manage daily patient referrals and intake operations in our Texas service area. You will help establish intake policies, enter charts into EMR, and maintain ongoing census and care plans.

The ideal candidate has 3–5 years of health care experience, knowledge of CHAP standards, and a valid driver's license. This full-time role offers competitive pay and comprehensive benefits.

Qualifications

• Degree in health care or related field preferred.
• 3–5 years of health care experience, hospice preferred.
• Admissions/intake and customer service experience required.

Responsibilities

• Direct all daily patient referral and intake operations including establishment and implementation of intake policies.
• Initiate chart into EMR and maintain workflow.
• Maintain census, pending admissions, red flag, journey's end and care plan meetings on stand up board.
• Ensure compliance with all state, federal and CHAP applicable referral/intake regulatory requirements.
• Establish and maintain positive working relationships with current and potential referral sources.
• Gather, collate, and report referral statistics including key customer referral trends.
• Participate in strategic planning including identifying opportunities for additional services.
• Maintain knowledge of company contractual relationships and ensure patients are admitted according to contracts.
• Participate in quality improvement teams and activities.
• Maintain knowledge of community resources and assist referral sources in accessing them.
• Timely submission of all documentation.
• Performance of other duties as required.
