Job summary
Heart to Heart Hospice is seeking an Intake Coordinator to manage referral and intake operations, shaping policies to ensure smooth admissions.
The role requires healthcare experience, familiarity with CHAP standards, government regulations, and EMR systems, plus a valid driver's license.
You will collaborate with referral sources, report statistics, and participate in strategic planning to improve services for patients and families.
Qualifications
- Experience in admissions/intake and customer service
- Knowledge of CHAP standards and government regulations
- Experience with EMR software and healthcare operations
Responsibilities
- Direct daily patient referral and intake operations, establish intake policies
- Initiate chart in EMR and maintain workflow
- Maintain census, pending admissions, red flags, and care plan meetings on the stand-up board
- Ensure compliance with state, federal and CHAP intake regulations
- Build relationships with referral sources and monitor community perception of Heart to Heart Hospice
- Gather and report referral statistics and trends
- Participate in strategic planning to identify new or improved services
- Ensure admissions align with contract provisions
- Participate in QA/PI teams and activities
- Know community resources and assist referrals in accessing them
- Ensure timely submission of all documentation
- Perform other duties as required
