Qualifikationen

• High school diploma or equivalent required; healthcare education preferred.
• 1+ year in healthcare administration, medical office support, or intake roles preferred.
• Experience in hospice, home health, hospital, or SNF is a plus.
• Strong organizational and time-management skills with attention to detail.
• Proficient with computers, EMR systems, email, and Microsoft Office.
• Knowledge of HIPAA and patient confidentiality; ability to work independently.

Aufgaben

• Receive, review, and process hospice referrals from hospitals and clinics.
• Verify patient information and collect required medical records and paperwork.
• Enter patient data into EMR and related systems; maintain accurate records.
• Communicate referral status and next steps to sources, patients, and teams.
• Coordinate with clinical staff for timely evaluations and admissions.
• Track referrals, follow up on missing documents, and escalate delays.
• Verify insurance eligibility and assist per procedures; maintain confidentiality.
• Monitor activity and produce intake documentation; adhere to HIPAA and policies.
