You will coordinate insurance verification, liaise with referral sources, and ensure accurate EMR data entry while supporting the sales and clinical teams.

The role requires strong communication, attention to detail, and a commitment to Affinity Hospice values and community presence.

Qualifications

• Diploma or degree in practical or vocational nursing or related field.
• Current CPR certification.
• Valid driver’s license and a reliable insured vehicle.
• Previous home health or hospice experience preferred.

Responsibilities

• Oversee all aspects of the patient intake process to admit patients timely.
• Maintain relationships with referral sources and clinical teams.
• Verify insurance and obtain necessary authorizations for admissions.
• Document intake activities in the EMR and communicate missing information.
• Assist with community education and participate in sales calls as needed.
• Represent the organization at community events and maintain contract compliance.
