Job summary

Affinity Health Management is seeking a dedicated Registered Nurse to plan, organize and direct hospice care in community settings. You will assess patients, develop care plans, administer medications and educate patients and families about self-care and comfort measures.

The ideal candidate has NLN-accredited nursing education, current state RN licensure, and 1–2 years of nursing experience in home health, hospice, or public health.

Qualifications

• Graduate of NLN-accredited school of nursing.
• Current RN license in the state.
• Experience in home health, hospice, or public health is preferred.
• Ability to plan and direct patient care with families.

Responsibilities

• Complete and update comprehensive patient/family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient/family involvement.
• Administer medications and treatments as prescribed.
• Coordinate care as Case Manager when assigned.
• Educate patients and families on self-care, medications, and pain management.
