Job summary

VITAS Healthcare Home Health Aide (HHA) supports hospice patients at home, delivering personal care under the nurse's direction and augmenting family care.

Responsibilities include assisting with hygiene, meal planning, ambulation, and basic treatments as taught by the nurse; documenting patient status and changes; and maintaining a safe environment. Must have HS diploma, 75 hours HHA training, and complete required competency evaluation.

Qualifications

• High School diploma or equivalent required.
• Must have 75 hours of HHA training per 42 CFR.
• Competency Evaluation program completion required.

Responsibilities

• Assist with basic personal care and hygiene per assignment.
• Assist with meals planning and feeding as needed.
• Assist with ambulation and mobility under nurse supervision.
• Monitor and report changes in patient condition to nurse.
• Record vital signs and document care provided.
