Job summary

VITAS Healthcare is seeking a Home Health Aide (HHA) to provide personal care and support to hospice patients in accordance with the plan of care. The role involves assisting with hygiene, meals, mobility, and basic treatments under nurse supervision.

The HHA will document patient status, report changes, and participate in at least 12 hours of in-service training per calendar year to maintain competency. This is a non-exempt, patient-facing position requiring reliable home-based service delivery.

Qualifications

• Must read, write, and follow directions.
• Willingness and ability to travel per job requirements.
• Evidence of 75 hours of training and competency evaluation as required by regulation.

Responsibilities

• Assist patients with personal hygiene and daily living activities.
• Plan and prepare meals; assist with feeding as needed.
• Assist with ambulation and mobility.
• Follow care plan and report changes to the VITAS Nurse and interdisciplinary team.
• Maintain infection control and document patient status and care given.
