Job summary

VITAS Healthcare seeks a Home Health Aide (HHA) to provide compassionate in‑home personal care for hospice patients under the direction of a VITAS nurse. The role includes assisting with hygiene, meals, mobility, and monitoring patient status according to the plan of care.

Daily responsibilities also involve basic treatment support, documentation, and caregiver education, while maintaining patient comfort and safety in a home setting.

Qualifications

• Must be able to read, write, and document patient information accurately.
• Completion of required Home Health Aide training and certification per CFR standards.
• Experience following a prescribed plan of care under supervision.

Responsibilities

• Assist patients with personal hygiene and daily living activities.
• Plan and prepare meals and assist with feeding as needed.
• Support patient ambulation and mobility as directed.
• Assist with treatments and medications as taught by nurse and physician per plan of care.
• Record vital signs and communicate changes to the nurse and interdisciplinary team.
