Job summary

VITAS Healthcare is seeking a Home Health Aide (HHA) to provide personal care and supportive services to hospice patients in their homes, working under the supervision of a VITAS Nurse within the interdisciplinary team.

The HHA assists with activities of daily living, nutrition, mobility, and basic treatments as prescribed. A High School diploma or equivalent, and state-required HHA training/competency are required; travel to patient homes is expected.

Qualifications

• Must have High School graduate or equivalent.
• Must meet personnel qualifications of a Home Health Aide as specified in 42 C.F.R. Section 484.4 and Section 484.36.
• Documented evidence of 75 hours of training as specified in 42 C.F.R. Sections 484.36 (a)(1).
• Documented evidence that Home Health Aide has successfully completed a Competency Evaluation program as described in 42 C.F.R. Section 484.36(b).

Responsibilities

• Assists patients with personal care and maintains good personal hygiene.
• Plans and prepares meals; assists with feeding when needed.
• Assists with ambulation and mobility.
• Performs treatments as ordered by physician and supervised by nurse within plan of care.
• Reads, records temperature, pulse and respiration.
• Maintains safe and healthy environment and reports changes to nurse.

Education

High School graduate or equivalent
