Job summary

VITAS Healthcare is seeking a Home Health Aide (HHA) to provide personal care and services to hospice patients in the patient's home, augmenting care provided by family or other caregivers. The HHA will perform only assigned tasks and support activities as outlined in the care plan.

The role requires adherence to training standards, ability to read and record vitals, and collaboration with the VITAS Nurse and interdisciplinary team to ensure patient comfort and safety.

Qualifications

• High School graduate or equivalent preferred.
• Meets personnel qualifications of a Home Health Aide as specified in 42 C.F.R. Section 484.4 and Section 484.36.
• Documented evidence of 75 hours of training as specified in 42 C.F.R. Sections 484.36 (a)(1).
• Documented evidence that Home Health Aide has successfully completed a Competency Evaluation program as described in 42 C.F.R. Section 484.36(b).

Responsibilities

• Performs only those personal care activities contained in a written assignment.
• Assists with personal hygiene and feeding as needed.
• Assists patient ambulation.
• Assists with treatments as ordered by physician and taught by nurse.
• Reads, records vital signs, and reports changes to nurse and inter-disciplinary team.
• Maintains safe and healthy environment and provides basic emotional support.
