Qualifications

• Provide personal care to patients per physician orders and regulations.
• Assist with activities of daily living and maintain patient safety.
• Accurately document services and report changes to RN/Clinical Manager.

Responsibilities

• Provide emotional and physical support to terminally ill individuals and families.
• Assist with bathing, grooming, dressing, ambulation and transferring.
• Plan meals, assist feeding, light housekeeping and linen changes.
• Provide companionship and life enrichment activities.
• Document services daily and report changes to RN Case Manager.
