Job summary

Beth Israel Lahey Health at Home is seeking a dedicated Home Health Aide to support patients in their homes. You will assist with hygiene, mobility, medication cues, meal support, and vital signs, while maintaining confidentiality and following CHHA policies.

Training and BLS certification are required, with a preference for applicants with prior CHHA experience. The role emphasizes compassionate patient care, documentation in EMR, and collaboration with the care team.

Qualifications

• Must have completed a basic training program approved by the Mass. Association of Homemaker and Home Health Aides and possess the certificate.
• Must have active American Heart Association BLS or complete BILHAH BLS competency if AHA BLS is not available.

Responsibilities

• Assist patients with hygiene, grooming, including shower, tub or bed baths and basic care.
• Assist with toileting using bed pans as needed.
• Assist with safe transfers using gait belts or lifting equipment.
• Assist patients with walking using walkers, canes or wheelchairs.
• Perform ROM exercises and proper positioning.
• Cue self-administered medications.
• Assist with meal preparation and feeding when required.
• Observe, report, and document patient status and care.
• Take and record vital signs as directed.
• Practice infection control procedures.
• Document patient status in EMR; report changes to supervisor.
• Maintain a clean, safe home environment for patients.
• Recognize emergencies and contact caregivers and supervisor.
