Job summary

Hebrew SeniorLife Hospice Care seeks an on-site Home Health Aide to provide compassionate personal care to hospice patients in their homes under physician orders and state regulations. You will assist with daily living activities, nutrition, safety, and comfort, while offering companionship and life-enrichment activities.

You will document care provided, monitor patient status, and escalate changes to the RN Case Manager/Clinical Manager.

Qualifications

• HHA or CNA credentials required and renewal of license.
• Ability to document services accurately per Hospice policies.
• Ability to perform personal care tasks with compassion and professionalism.

Responsibilities

• Provide emotional and physical support to terminally ill patients and families.
• Assist with activities of daily living (bathing, grooming, dressing, mobility).
• Assist with meals, light housekeeping, and environment safety.
• Provide companionship and engage in life-enrichment activities.
• Document services rendered promptly and accurately in patient records.
• Report changes in patient condition to RN Case Manager/Clinical Manager.
