Job summary

Beth Israel Lahey Health at Home – North Shore is seeking a Hospice Aide to provide compassionate personal care and homemaker services to patients in their homes across Beverly, Peabody, Stoneham & Wakefield areas. The role follows an IDT plan of care and requires active patient engagement.

Responsibilities include assisting with hygiene, mobility, meals, and medication cues, while maintaining infection control and documenting patient status for the hospice nurse.

Qualifications

• Valid MA Certified Nursing Assistant (CNA) certification OR Home Health Aide (HHA) certificate required.

Responsibilities

• Provide personal care and homemaker services as directed by the IDT plan of care.
• Assist with personal hygiene including showers, grooming, and oral care.
• Support patients with toilet needs and safe transfers using gait belts or lifting equipment.
• Monitor and document patient status and vital signs as directed.
• Assist with walking and mobility using assistive devices.
• Support medication reminders and basic administration cues.
• Prepare meals and assist with feeding when required.
• Maintain a clean, safe home environment and observe for changes in condition.
• Report changes to the hospice nurse case manager.
