Job summary

Elite Hospice, a part of LHC Group within the Optum family, seeks a Home and Community Bases Services Aide to help individuals stay in their own homes. You will provide personal care and assist with daily living activities, while supporting household tasks and accompanying clients to appointments.

Responsibilities include monitoring client condition, vital signs, and incidents; maintaining confidentiality and using EVV.

Qualifications

• Current driver's license, vehicle insurance and reliable transportation or access to public transport.
• CPR certification or ability to complete within 90 days of hire.
• Household tasks assistance and personal care assistance required.
• Flexible hours and ability to work independently.
• 6+ months of home care experience preferred.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
